RN Case Manager – Pediatrics (Home Health)

Location: Brooklyn, NY

Schedule: Monday–Friday | Day Shift | Flexible Hours

Salary: $110,000–$120,000 + travel reimbursement

Bonus: $15,000 sign‑on

Role Summary

The RN Case Manager – Pediatrics provides one‑on‑one home‑based nursing care to pediatric patients, primarily ages 0–2. This role focuses on assessments, care planning, education, and coordination — no office time required.

What You’ll Do

Complete initial assessments, recertifications, and manage pediatric caseloads

Develop and implement individualized pediatric care plans

Provide clinical care and parent/caregiver education

Collaborate with an interdisciplinary care team

Document care accurately and timely

What You’ll Need

Active NY RN license (or ability to obtain)

Pediatric inpatient or pediatric clinical experience

Strong assessment, communication, and organizational skills

Ability to work independently in a community setting
